Not sure how else I can communicate the problem but if there's something more I can provide please let me know. I don't know if it's any help at all but I found the diagnostic logs feature in the help menu and I've included that file here. If it was just a playback issue that would be one thing, but I can't tell you how many renders I've had to delete and repeat because of this problem. Sometimes I can adjust the location/duration of the fade and it'll come back, other times I have to completely remove it and apply it again. Seems like just about any time I use a fade (in, out, crossfade, any type), regardless of whether I use the audio transition or the fade handle on any clip or a combination of both, if afterwards I make any edits whatsoever to those clips on the edit page there's a significant chance the fade will not play back.
